PAYMENT INFORMATION
All fees for orthodontic treatment are determined by the treating orthodontist on
an individual basis using factors such as severity of the case and length of treatment.
Our office is pleased to be able to offer a free consultation including diagnostic
records so that our patients can have a clear understanding of the orthodontic treatment
plan and financial plan that is being proposed for you or your child.
In order to make orthodontic treatment affordable for our patients, Drs. Davis &
Nyczepir offer several payment options:
A no-interest, no-finance charge in-house payment plan consisting of an initial
down payment followed by monthly payments that are payable on the first of the month.
A payment in full choice reflecting a 5% discount for out-of-pocket treatment fees
over $1,000 when paid at the start of treatment.

Payment can be made by cash, personal check, money order or Visa, MasterCard and
Discover cards. If you would like to make a payment by phone or schedule a recurring
