COVID-19 Health Questionnaire

We ask that you fill them out before your appointment if possible. If you are unable to print them at home, we will have them available for you at the office.

Dear Patients and Parents,

                 We are excited that we are finally able to open our doors to our orthodontic family. Our #1 priority will be to provide the highest quality orthodontics in a safe environment to our patients and our team.  That being said, we will continue to adhere to strict CDC/OSHA infection control guidelines and will be implementing some additional changes to make your visits as smooth as possible.  Please read the following carefully to help us achieve that goal.

1) Only healthy patients who can answer NO to the following 4 questions will be seen :

  • Have you been ill within the last 14 days?
  • Have you had a fever within the last 14 days?
  • Have you traveled outside of the country in the last 14 days?
  • Have you been in contact with a COVID-19 patient or been exposed?

If you answer yes, we will kindly reschedule your appointment when you are well.

2) Prior to leaving your home or office for your appointment, please brush your teeth and make them “sparkle” before your orthodontic visit. We will not have any public facilities. The toothbrushing areas, waiting rooms, game rooms and bathrooms will all be closed. Please wear your own mask to your appointment (if available). If you do not have a mask, we’ll provide one.

3) Our schedule and patients will be spaced out accordingly to allow for social distancing. Only patients may enter the office and we’re asking that parents and other family members remain in the car.

4) Upon arrival, please call our office and advise the receptionist that you are here. An assistant will then meet you or your child at the door to begin their appointment.

  • We will ask patients or parents to fill out a new COVID-19 informed consent

5) Once inside, we will:

  • Take the patient’s temperature ** Any patient with a temperature of 100.4 ( recommended by the CDC) or higher will be re-appointed.
  • Have the patient rinse with a peroxide mouth rinse
  • Sanitize the patients hands with hand sanitizer
  • Perform the scheduled appointment

6) Upon completion of the appointment, the patient will be dismissed and escorted to the front door. If the patient is a minor, we will walk them to the vehicle and give you a treatment update at that time. We will then schedule your next appointment by phone.

** Please notify us if you have anything broken, prior to your appointment, so that we can reschedule you for the regular and repair appointment to be completed at the same time. **
